Urgent Notice Regarding 6th Grade
Immunization Requirement
This year the New Jersey
Department of Health and Senior Services revised its immunization
requirements for incoming 6th Grade students. Students are
required to have a TDAP (Tetanus, Diphtheria & Acellular Pertussis)
vaccine and a Menactra (Meningitis) vaccine by September 1, 2008.
This applies to students who are at least 11 years old and attending 6th
grade or an ungraded special education program. We appreciate your
cooperation in fulfilling this new requirement. Kindly provide
documentation from your child’s physician with the name of the vaccine
and the date it was administered.

Urgent Notice Regarding Pre-School Immunization Requirement
This year the New Jersey Department of Health and Senior Services
revised its immunization requirements for all Pre-School students.
Pre-school students are required to have a Pneumococcal vaccine by
September 1, 2008 and one dose of Influenza vaccine administered
between September 1, 2008 and December 15, 2008. The Influenza
vaccination is an annual requirement. This applies to all students who
are attending a preschool program. We appreciate your cooperation in
fulfilling this new requirement. Kindly provide documentation from
your child’s physician with the name of the vaccine and the date it
was administered.
